Guardian Care Services is dedicated to the provision of health services at our patients’ homes. We have been serving the area since 2000. Our commitment to the community’s well being goes further than the patients we serve. We promote healthy living and preventive medicine to our staff, partners and referral sources.

Physical Therapy

Assist to our patients in recovering, restoring and maintaining strength and endurance, and restore musculoskeletal abilities. From Arthritis, fractures, sprains,with limited ambulation, unsteady gait, our therapist are specialized in helping patient restore their functional limitations, and provide confidence in their ability to ambulate.

Occupational Therapy

Skills include patient treatment for those impaired due to a physical illness, emotional disorder, congenital or developmental disability, or the aging process. Our program combines a strong employee’s curriculum and opportunities for practical application in fieldwork experiences.

Speech Therapy

Assists in improving swallowing disorders and the communicative activities of daily living.

Medical Social Workers

Search and explain benefits, find better ways to Patient’s health maintenance, facilitate long term planning to promote independence and provide opportunities to take advantage of a variety of community resources designed to assist the elderly with their personal concerns and help alleviate their troubles.

Care Planning

A comprehensive plan of care is formulated after an initial assessment with a registered nurse or therapist in the patient’s home. RN Case Managers supervise and coordinate care with the patient, caregiver, members of the treatment team, and the physician.
